Candidates fulfilling the eligibility criteria as specified above may attend the Walk-in Interview on the above date, time and venue with the duly filled in Application Form (Annexure-1) with two (2) recent passport size colour photograph and the following certificates/documents in original along with self attested photocopies of the certificates/documents:

a) Proof of Date of Birth (Class X / Matriculation certificate of recognized Board or Council /Birth certificate issued by the competent authority).
b) Class XII Mark sheet
c) MBBS Mark Sheets of all years
d) MBBS Degree/Passing Certificate
e) MBBS Registration Certificate (Medical Council of India (MCI) or National Medical Commission (NMC) or State Medical Council (SMC).
f) Internship completion certification (For GDMOs)
g) Valid practitioner license.
h) Experience Certificate showing relevant post qualification experience from the employer.
i) Photo Identity Proof (either Aadhaar Card, PAN Card, Driving License, Passport, Voter ID).
j) Address proof (Voter ID, Driving License, Aadhaar Card or others).
k) Category/Caste Certificate SC/ST/OBC (NCL)/EWS and PwBD Certificate (if applicable).
